Assistant Service Coordinator

This position will be 20 hours/week.

Humility Homes & Services, Inc. (HHSI) is seeking a qualified individual to work part time in our Supportive Services for Veterans and their Families Program.

General Description

The Assistant Service Coordinator is responsible for providing support and case management to participants in Humility Homes & Services, Inc. SSVF program. Their primary focus is assisting people to obtain and remain in permanent housing by providing transportation assistance and housing advocacy to potential landlords/property managers as needed by Service Coordinators and the Veteran Services Supervisor.

Case management:

  • Assist Service Coordinator team with delivery of payments to indicated vendors

  • Identify housing options for program participants to review, as needed.

  • Complete other mandated assessments per specific grant agreement or program policy and procedures

  • Complete VISPDAT with any program participant who is literally homeless, as directed

  • Transport program participants to appointments, apartment showings and any other necessary locations based on their ISP

  • Connect participants to mainstream benefits and assists them in applying when applicable

  • Liaison with landlords to either help a participant obtain housing or address any concerns once the participant is housed

  • Provide life skills training like laundry, navigating the bus system etc. for program participants

  • Assist maintenance staff in coordinating and delivering items from Fresh Start Donation Center to participant’s rental unit, as assigned.

  • Provide crisis intervention and advocacy for participants, as needed

Documentation:

  • Complete daily case notes, service transactions, and contacts and upload them to ServicePoint.

  • Assist in obtaining and maintaining current income verification, eligibility and services provided as required for specific grant agreement

  • Maintain releases of information for all participants and external providers when applicable

  • Complete fund requests for participant needs when applicable, in collaboration with Service Coordinators

  • Refers participants to other programs at HHSI and community partners for which they are eligible, along with supportive documentation

  • Other duties as assigned
